CONTINUING REVIEW FORM

In accordance with federal regulations and guidelines, animal protocols must be reviewed annually by the Monmouth University Institutional Animal Care and Use Committee (IACUC). Please complete and submit this form to the IACUC for review. Any changes in the protocol to include changes in the species or number of animals used, changes in experimental procedures, and/or changes in personnel must be reported.
