Student Support Self-Referral Form

Empowering students through self-advocacy and unwavering support, students can fill out a Student Support Self-Referral form for mental health concerns, emergency financial need, and access to basic needs assistance.

Students are reminded, submissions are not monitored 24/7 and should not be used in the case of an emergency.

If you believe this is an emergency situation, please contact MUPD at 732-571-4444 and if you are off-campus, please dial 988 for the Crisis Lifeline or 911 for emergency services.

What to Expect Once a Self-Referral Form Has Been Submitted

Once a Student Support Self-Referral form has been submitted, an auto-generated email will be sent to the student confirming that their form has submitted. Outreach from a member of Monmouth University’s CARE Team will be conducted via email, requesting to meet with the student one-on-one to help the student access the tools and resources needed to resolve their current situation

During this meeting, the student will be asked a series of questions to help better understand the student’s unique situation and what resources may be most beneficial.
